Junior Officials Request Form
Ottawa South Basketball Association (OSBA)
This form is to be used by coaches and team general managers to request game officials through the Ottawa South Basketball Association.

All requests will first be filled by OSBA Junior Officials.

If we are unable to staff your game fully, we may engage support from our partner associations, including Nepean Basketball and the Ottawa Shooting Stars.

Please ensure all information is complete and accurate. This will allow our officials coordinator to confirm assignments and communicate with you quickly.
Once submitted, your request will be added to our scheduling system. You will be contacted to confirm your officials and any related details.
